This is simply meaning that on approach to or at a junction the learner failed to look carefully enough and act upon what they saw.
The reason Learners don't look carefully enough is often linked to the speed at which they approach the junction or they don't look at all until they at the line.
Junctions are not always easy, as parked cars and street furniture often obscure the view and if you are emerging from a minor road you have to take great care.
